Privacy officer
Axis Integrative Health Consulting is a business based in Montreal, Quebec. Myriam Elias, founder, acts as the person in charge of the protection of personal information under Quebec’s Act respecting the protection of personal information in the private sector.
For any question about this policy, or to exercise your rights, write to info@axisihc.com.
The information we collect
Information you give us. When you book a discovery call or write to us, we collect your name, email address, phone number if you provide one, and the content of your message or of your answers in the booking form.
Information collected automatically. Like most websites, axisihc.com records technical browsing data: IP address, device and browser type, pages visited, date and time of the visit. Beyond what is strictly necessary for the site to work, this data is collected only with your consent, given through the cookie banner.
The site has no payment form and collects no banking information.
Why we collect it
Your information is used only to:
- answer your questions and requests
- schedule and hold your discovery call
- provide and follow up on the services you ask for
- understand how the site is used and improve it, where you have consented
- meet our legal and professional obligations
We never sell, rent or trade your personal information. No decision concerning you is made on a purely automated basis, and no profiling is carried out.
Cookies and the consent banner
Cookies are small files stored on your device. Only those strictly necessary for the site to function are set by default. All others, including analytics cookies, those measuring how our Google ads perform and those loaded by the booking calendar, are activated only after you consent. Advertising cookies are only used to know whether an ad led to a booking: the site does no retargeting or personalized advertising.
The site’s consent banner is powered by Consorm, a Quebec-based consent management solution built for Law 25 and hosted in Quebec. It blocks trackers until consent is given and keeps a record of your choice.
You may change or withdraw your consent at any time by reopening the banner from the site. You can also set your browser to refuse cookies, keeping in mind that some features, such as the booking calendar, may then no longer display.
Third parties we work with
A few providers are involved in running the site. Each one only has access to the information its function requires.
- Calendly, for scheduling. The calendar is embedded on the contact page: the information you enter there is processed by Calendly, whose servers are located in the United States.
- Consorm, for the consent banner and the record of your choices, hosted in Quebec.
- Google, for the reviews shown on the home page. They come from the Axis Google listing and are fetched by our server. No information about you is sent to Google when they are displayed.
- Google Analytics and Google Ads, to measure site traffic and to know whether an ad led to a booking. These measurements are only activated with your consent, and the data is processed by Google on servers located in the United States.
- The site’s hosting provider, which keeps technical logs needed for the security and stability of the service.
Disclosure outside Quebec
Some of these providers process information outside Quebec, notably in the United States. Before using such a service, we assess whether the information receives adequate protection there, and we limit what is transmitted to what is strictly necessary.
Retention and destruction
Your information is kept for as long as needed for the purposes it was collected for, then destroyed or anonymized. Exchanges that do not lead to an engagement are destroyed no later than twenty-four months after the last contact. Information we must keep under a legal or professional obligation is retained for the period that obligation requires.
Your health information
No health information is collected by this website. What you share during naturopathy or clinical hypnotherapy sessions is recorded in your client file, outside the site, and is protected by the practitioner’s confidentiality obligations as well as by the commitments given to you when your file is opened.
We ask that you not send detailed health information by email or through the booking form. The discovery call is the right moment to discuss it.
Security
We take reasonable measures to protect your information: an encrypted connection across the whole site, access limited to those who need it, protected accounts, and providers chosen for their security practices. As no transmission over the Internet is entirely foolproof, we cannot guarantee absolute security.
Your rights
The law gives you the right to:
- access the personal information we hold about you
- have it corrected if it is inaccurate, incomplete or ambiguous
- withdraw your consent at any time
- ask that information no longer justifiably kept be deleted
- receive, in a structured and commonly used technological format, the computerized information you provided to us
- file a complaint with the Commission d’accès à l’information du Québec
To exercise any of these rights, write to info@axisihc.com. We respond within thirty days of the request. Proof of identity may be requested to confirm the request comes from you.
Confidentiality incidents
Should an incident involving personal information occur, we take reasonable measures to limit its consequences and prevent it from happening again. If the incident presents a risk of serious injury, we notify the people concerned and the Commission d’accès à l’information, and the incident is entered in our register.
Minors
The site and the services of Axis are intended for adults. Information concerning a person under the age of fourteen is collected only with the consent of the person having parental authority.
Changes to this policy
This policy may be updated to reflect a change in our practices or in the law. The version shown on this page is always the one in force, and the date it was last updated appears at the top of the page.
